In particular, if you want to use the form, you need to have a form-processing program set up on your server, for which you must enter the URL in the 'action' attribute of the <form> tag. If your server has a recent version of Perl installed, I recommend 'FormMail' from The London Perl Mongers (it's much more secure and reliable than any other free scripts I've seen). If you can't set up scripts on your own server, then a remotely-hosted form processor such as that provided by Response-O-Matic is a good alternative. You may need to change the 'name' and 'id' attributes of the form elements to suit your own needs or the form processor program you are using: I have named them so that they will work with London Perl Mongers 'FormMail'.
The style sheet already provides style for <input type="text"> and <textarea> fields, and for buttons through the addition of a class attribute, e.g. <input type="submit" value="Send Form" class="button" />. If you want to add radio buttons, checkboxes, etc. you'll have to style them yourself!
